What’s Included in Your Membership?

Our membership covers all of your primary care needs. We have three memberships available: Hive (in-office), Nest (in-home) and Honeycomb (mother/newborn). We will work to keep you out of emergency rooms, urgent care centers, and minute clinics. Your child is best cared for by a doctor who knows them, their history, and their unique needs. This includes coverage for all of the following:

  • Routine well visits

  • Unlimited sick visits (acute and chronic care)

  • Remote visits/virtual care

  • Routine vaccinations

  • School/camp/sports physical forms 

  • Management of minor lacerations (stitches, staples, glue) 

  • Developmental and behavioral screenings 

  • Counseling and guidance (nutrition, development, sleep, behavior, parenting, and more)

  • Initial lactation consult for newborns

  • Evaluation and medical management of chronic pediatric conditions such as eczema, asthma, ADHD, depression, and anxiety

Honeycomb

Includes prenatal visit and Hive membership for the first 3 months of life with in-home visits included for the first month.

Honeycomb: one-time payment. Includes registration fee and two in-home visits.

Honeycomb At-Home: one-time payment. Includes registration and all in-home visits.

 
 
 
 

*There is a one time non-refundable enrollment fee of $100. This fee is waived for previous patients of Dr. Coleman who are transitioning to her DPC practice. Family maximum of $400/month for Hive members and $700 for Nest members.

*Families who pay annually will receive a 5% discount. We have discounts available for families with children in foster care, teachers, medical students, and residents.

*There are a limited number of financial assistance spots available to families with an annual household income of less than $60,000. Please contact us for more information.

*There are a limited number of openings available for one-time visits for various needs. Please contact us to discuss availability and pricing.

*House calls outside a 20 minute drive from our office will be charged an additional fee based on distance. House call availability is limited and not guaranteed.
